As a home care provider, we view the protection of our patients’ rights as an integral part of our services. These rights go hand in hand with the principles and philosophy Revival strives to achieve. We provide all of our patients with a full list of their rights upon initiation of care.
Dignity and Respect. You have the right to:
- Ethical Standards – to have a relationship with our staff that is based on honesty and ethical standards of conduct.
- Non-Discrimination – to receive competent quality services from the agency regardless of your race, color, religion, national origin, age, sex, sexual preference, marital status, ability to pay, source of payment, or handicap.
- Respect for your property – to have your property treated with respect.
- Service – to expect that our personnel are properly qualified to provide your care and that you will know the names and titles of the agency personnel who serve and supervise you.
- Communication – to have the agency staff communicate to you in a language or form understandable to you. If a language barrier exists, the agency will provide assistance to assure your understanding.
- No reprisals – to voice grievances without fear of discrimination or reprisal for having done so.
Decision-Making. You have the right to:
- Information about your care – to be informed as soon as possible of the nature and purpose of the care to be provided, types of caregivers, planned frequency of services, expected outcomes, barriers to treatment, and changes to your care.
- Plan your care – to actively participate in the planning of your home care services and goals associated with this plan.
- Accept or refuse treatment – to be informed in writing of your rights under State law to make decisions concerning medical care. You have the right to accept or refuse treatment and formulate advance directives.
- Implement advance directives – to be informed in writing of policies and procedures for implementing advance directives. You will be informed if we cannot implement an advance directive.
- Compliance with your wishes – to have health care providers comply with advance directives in accordance with State laws
- Receive care without conditions – to receive care without condition or discrimination based on whether or not you have executed advance directives.
Privacy and Confidentiality. You have the right to:
- Confidentiality – to confidentiality of your medical records as well as information about your health, social and financial situation.
- Authorize release of information - to request that we release written information about you only as required by law and to be informed of the procedures for disclosure of this information.
