Diabetes Program
What is it? Revival’s Diabetes Program includes educating the patient and family about medication, how to monitor blood sugar levels and when to seek medical attention and we coordinate the care with the patient’s physician.
How does it work? Revival’s Registered Nurses are trained to teach patients and their family members how to manage this chronic disease. We instruct patients on the importance of nutrition and exercise, how to monitor their own blood sugar levels and how to prepare and administer insulin and oral hypoglycemic medications. Revival’s nurses teach patients what they need to know about how to control their disease and what factors will affect blood sugar levels. Our goal is to empower our patients with the knowledge they need so they can live long, healthy lives without any of the complications associated with the disease.
Who should use this service? People of all ages who have received a diagnosis of Diabetes. From children to senior citizens, when patients understand how the disease works and what they can do to control it, Diabetes becomes a manageable illness that does not necessarily affect the patient’s ability to live a normal lifestyle.
Who provides this service? Revival’s Diabetes Team consists of a Registered Nurse who is a Certified Diabetes Educator (CDE) and field nurses who make regular home visits. The CDE will visit patients periodically, to ensure that they understand fully how to monitor their disease and care for themselves. In addition, the Diabetes Team consults with the patient’s physician regularly, providing up to date information about the patient’s condition and progress.
