Each patient we serve, receives a personal Plan of Care carefully structured to ensure that you will receive the kind of care you need to restore you to health.   Revival Home Health Care works closely with your doctor so you receive the best possible care in the comfort of your own home.

Evaluation. Our professional and experienced intake nurses will evaluate your medical needs and provide an experienced registered nurse or therapist to come into your home. We consult with your family members and physician to come up with the best plan of care to suit your needs. To ascertain what care plan will be best for you we will ask your family member or doctor:

• What injuries, illnesses or disabilities does the patient have?
• What physical functions need to be improved and how do they affect the patient’s needs?
• What is the patient’s current mental state and how will that affect quality of life?
• What insurance does the patient have?

Revival’s Individual Plan of Care. Revival Home Health Care works with you and your physician.  Our nurse comes to your home and formulates a plan based on information we receive and our assessment of your needs.  The plan is tailored to help you achieve the optimal level of physical and emotional well-being.  Your care plan is regularly reviewed, and when necessary, modified to ensure that you continue to receive unsurpassed care that may include the following intervention and goals:
