Adult Care Program

What is it? Adult Care services complement and ensure continuity of care.   Revival coordinates Adult Care through consultations with patients’ physicians and careful patient monitoring.

How does it work? The Adult Care team, represented by our intake nurse, provides initial evaluation of the patient. After assessing the condition and needs of the patient, the team coordinates care, monitors progress closely and makes adjustments as needed.  The result is, our patients feel well cared for and supported throughout the healing process.  Revival makes every attempt to be non-intrusive and supportive of family and patient.  Adult Care staff is equipped with the telehealth system, state of the art monitoring equipment that ensures accurate vital readings and record keeping. Revival has the most up to date computerized technology available in the home care industry.

Who should use this service? Adult Care is best suited to adult patients of all ages who require continuous care at home.

Who provides this service? The adult care program at Revival Home Health Care is staffed by Registered Nurses, Licensed Practical Nurses and home health aides, who are sensitive to the individual needs of patients and their families.
